　　A.如果投標人為境內(nèi)注冊公司，投標人需提供合法有效的企業(yè)法人營業(yè)執(zhí)照、稅務(wù)登記證及組織機構(gòu)代碼證或證照合一的營業(yè)執(zhí)照，投標時需提供原件掃描件(原件備查)；

　　A.The bidder within the customs territory shall provide valid independent legal person business license, tax registration certificate, organization code certificate, or combined certificate and business license. The scanned copy of the original shall be provided during bidding (the original copy for reference);
 

　　The bidder outside the customs territory shall provide valid certificate to prove valid business registration certificate.
 

　　B. 投標人應(yīng)為本次投標所投產(chǎn)品的制造商；如投標人為制造商同一集團負責(zé)銷售投標所投產(chǎn)品的全資子公司、控股子公司(投標人應(yīng)提供制造商或其上級公司證明上述事項的證明文件)，則視投標人為制造商。？投標產(chǎn)品由制造商不同工廠生產(chǎn)且使用相同品牌時，予以接受。
 

　　The bidder shall be the manufacturer of the bidding products.If the bidder is a wholly-owned subsidiary or holding subsidiary of the same group as the manufacturer responsible for sales of the bidding products, the bidder shall provide relevant evidence documents from the manufacturer or its superior company to prove the above matters, and in such case the bidder shall be deemed as the manufacturer. The bidding product produced by different factories of the manufacturer with the same brand shall be accepted.

　　A.投標產(chǎn)品制造商提供的熔體壓力變送器及接線盒須符合相應(yīng)的防爆標準，并取得防爆檢驗機構(gòu)的相應(yīng)防爆等級的《防爆合格證》，防爆等級為Ex db ⅡC T4 Gb/Ex ia ⅡC T4 Ga，《防爆合格證》應(yīng)由NEPSI、CQST、PCEC、IECEx、ATEX之一或國家認可的其他防爆認證機構(gòu)出具。
 

　　A.The melt pressure transmitters and junction boxes provided by the bidder manufacturer must comply with applicable explosion-proof standards and obtain the Explosion-Proof Certification? corresponding to the specified protection level. The certification must cover the explosion-proof rating of Ex db IIC T4 Gb/Ex ia IIC T4 Ga and be issued by one of the following entities: NEPSI, CQST, PCEC, IECEx, ATEX?, or other nationally recognized explosion-proof certification bodies..
 

　　B.投標人提供的熔體壓力變送器及其附件必須均取得相應(yīng)的國家權(quán)威機構(gòu)的功能安全認證，等級不應(yīng)低于SIL2。SIL證書應(yīng)由TUV、Exida、BV、北京儀綜所(ITEI)和上海自動化所(SITIIAS)之一的認證機構(gòu)出具。
 

　　B.The bidder-provided melt pressure transmitters and accessories must all obtain corresponding functional safety certification from nationally authoritative institutions, with a certification level no lower than SIL2. The SIL certificates shall be issued by one of the following certification bodies: TüV, Exida, BV, Beijing Institute of Instrumentation (ITEI), or Shanghai Automation Instrumentation Institute (SITIIAS).
 

　　(3)業(yè)績要求Requirement for Bidder's reference:
 

　　2015年1月1日至投標截至日(以合同簽署時間為準)，投標產(chǎn)品制造商在中國境內(nèi)有至少1個合同的應(yīng)用于聚碳酸酯裝置的熔融體壓力變送器供貨業(yè)績，且單個合同供貨數(shù)量不少于30臺。
 

　　同一集團全球范圍內(nèi)不同制造商(即同一集團旗下的全資子公司或控股子公司或制造工廠)制造的所投產(chǎn)品的業(yè)績，視為投標人所投產(chǎn)品的制造商的業(yè)績；投標人應(yīng)提供說明函，說明本次所投產(chǎn)品的制造商與滿足上述業(yè)績要求的產(chǎn)品制造商屬于同一集團。
 

　　From January 1st, 2015 to the deadline for submission of the bid (subject to the signing time of the contract), the manufacturer of the bid Melt Pressure Transmitter shall have at least one supply contracts performance of Melt Pressure Transmitter for PC Unit within P.R.China, and a single contract involved supplying no fewer than 30 Melt Pressure Transmitters.
 

　　The performance of the products submitted by different manufacturers within the same group (i.e., wholly-owned subsidiaries, holding subsidiaries or manufacturing plants under the same group) is regarded as the performance of the manufacturer of the products submitted by the bidder; the bidder should provide an explanatory letter to state that the manufacturer of the products submitted in this bid and the manufacturer of the products meeting the above performance requirements belong to the same group.
 

　　投標人須按規(guī)定格式提交業(yè)績表，并提交相關(guān)業(yè)績證明文件。業(yè)績證明文件包括：1)銷售合同和2)用戶簽字或蓋章的到貨驗收材料(到貨驗收單或調(diào)試驗收報告或其他可以證明合同項下貨物已經(jīng)到貨驗收的有效證明材料)。投標人所提交的業(yè)績證明文件必須至少體現(xiàn)以下內(nèi)容：合同簽署時間、合同簽署頁(國內(nèi)貿(mào)易合同應(yīng)有雙方蓋章，國際貿(mào)易合同應(yīng)有雙方簽字或蓋章)、制造商名稱、裝置名稱、貨物名稱、供貨數(shù)量。
 

　　若業(yè)績合同為年度協(xié)議，除提供年度協(xié)議外，還應(yīng)提供相應(yīng)的已到貨訂單，訂單內(nèi)容或編號應(yīng)與年度協(xié)議相關(guān)聯(lián)。同一個年度協(xié)議下提供1個或以上的訂單及與訂單對應(yīng)的到貨驗收材料均算為1個有效業(yè)績。
 

　　The bidder shall submit the performance record form in the prescribed format along with relevant supporting documents, which must include:sales contracts and delivery acceptance materials signed or sealed by the end-user (such as delivery acceptance forms, commissioning acceptance reports, or other valid documentation proving goods acceptance under the contract);all submitted proof documents must contain at minimum:the contract signing date, executed signature pages (with both parties' official seals for domestic contracts, or signatures/seals for international contracts), manufacturer's name, plant/installation name, description of supplied goods, and supplied quantity.
 

　　If the performance contract is an annual agreement, in addition to providing the annual agreement, corresponding received orders should also be provided, and the order content or number should be associated with the annual agreement. Providing one or more orders and corresponding acceptance evidence under the same annual agreement is considered as one valid performance.
 

　　未提交業(yè)績證明文件，或通過所提供的業(yè)績證明文件無法認定滿足上述業(yè)績要求的，均視為無效業(yè)績。
 

　　If no documentary evidence furnished, or documentary evidence furnished can not demonstrate the above performance requirements, the relevant reference will be deemed invalid.
